Preparing for your appointment

Coming prepared can save a lot of time. Please bring:
1. Specific detailed itinerary of trip
2. Record of previous vaccinations
3. Any forms travel agent or school program requires
4. International Certificate of Vaccination (if already have one, otherwise we will provide)
5. Picture ID
6. All current insurance cards
7. Payment for services, co-payments or deposits due at time of service
